Enrollment
Enrolling your child(ren) is easy!
Simply:
1. Read your Parent Handbook.
2 Completely fill out and sign the Enrollment Form.
3. Provide within 30 days of enrollment:
Form 3040 (yellow)-physical examination
and
Form 680 or 681 (blue)-immunization
record
You can obtain both from your child's pediatrician.
4. Drop off forms along with registration and supply
fees at your desired location.
A copy of the enrollee's parent/guardian's
driver's license will be made at this time.
5. Enjoy the peace of mind of knowing you've found
quality child care at an affordable price.
Below is a letter from the Department of Children and Families, commending Kids by the Hour.
