Enrolling your child(ren) is easy! 

Simply:

1. Read your Parent Handbook.
2. Completely fill out and sign the Enrollment Form.
3. Provide within 30 days of enrollment:
   1. Form 3040 (yellow)-physical examination and 
      
   2. Form 680 or 681 (blue)-immunization record
      (You can obtain both from your child's pediatrician.)
      
4. Drop off forms along with registration and supply fees ($100).
   ​A copy of the enrollee's parent/guardian's driver's license will be made at this time.
